Case Management & Advocacy
Integrating Case management and advocacy support with primary care or service is not only a good practice but it’s now widely considered an effective strategy for improving wellness for all people.
Our case management and advocacy program work with those we serve, both as a provider and as a facilitator of services our agency does not provide. The case manager specialist performs assessment of client’s needs and help link them to the available services and resources in the community.
Research shows several benefits associated with utilization of Case management support, but particularly case management and advocacy can result in better patient and client outcomes, better compliance with medical advice, and better patient self-management.
